By checking this box and signing below I hereby grant the City of Kamloops, including its employees and respected representatives that the City of Kamloops may authorize on its behalf, the right to photograph and take video footage of my child. 

I consent to the publication and use of these photographs and video footage, or images by the City of Kamloops including its employees and respected representatives for the promotion of fire safety and of the Junior Fire Chief program. I acknowledge the City’s right to crop or treat the photograph(s), video footage, or images at its discretion. I agree that I will not be compensated financially for the use of these photographs or recordings.

I agree that the photograph(s), video footage, or images that the City has arranged to be taken of my child shall remain the sole property of the City of Kamloops, and that the City has the full right to dispose of any or all of them in any manner whatsoever.

As the City of Kamloops proposes to act on this consent forthwith, I hereby release and discharge the City and its respective representatives, licensees, successors and assigns from all manner of actions, cause of action, debts, accounts, contracts, claims and demands whatever which myself or my heirs, executors, administrators or assigns can, shall, or may have at any time as a result of any act, matter or thing whatsoever arising out of or in connection with the consent and authorization given myself in this agreement.
